MyoMaps Job Aid The MyoMaps MR imaging application provides a means to generate pixel maps of the T1, T2 and T2* relaxation times in MR imaging examinations of the heart. Thus, the relaxation times, estimated using imaging sequences gated to the cardiac cycle, are encoded, in milliseconds, in each image pixel. This provides an additional layer of diagnostic information about changes in tissue composition, to potentially detect and quantify diffuse pathologies at an early state and monitor therapy. T1 and T2 mapping also offer optional image-based motion correction software to correct for in-plane motion between images. MyoMaps imaging parameters are located on the Inline > Cardiac Parameter Card. The software provides a interface to enable the evaluation of myocardial tissue T1 relaxation times. The so-called native T1 values of myocardial tissue may be estimated using “long T1” protocols provided in the Siemens tree. T1 mapping is performed using an ECG-triggered inversion recovery-based imaging technique. This technique involves the acquisition of singles hot TrueFISP images at different inversion times (TI) following an inversion pulse. The acquisitions are gated to the same cardiac phase, thereby enabling a pixel-based T1 value estimation in the myocardium. After the inversion, the magnetization following the T1 relaxation curve is repetitively sampled over several heartbeats, followed by a number of recovery heartbeats to allow for the magnetiza- tion to recover. For consistency, gradients are played out during these recovery periods without acquiring data. By combining several, typically two to three inversions with slightly shifted TI times within one imaging protocol, the relaxation curve is sampled in an interleaved manner, resulting in a sufficient number of points for estimating T1 values within a single breath-hold. T1 mapping is typically performed during a breath-hold and the use of navigator gating is not supported. Motion correction: An optional fast and fully automatic nonrigid registration algorithm is uti- lized to compensate for in-plane motion between the images. The T2 mapping technique is ECG triggered and uses a T2-prepared sequence combined with ei- ther TrueFISP or gradient echo readout to produce single shot T2-weighted images, each with a different T2 preparation time. The sequence will wait for several recovery heartbeats between each preparation to allow for full T1 regrowth. Gradients are played out during these recovery periods without acquiring data to ensure a consistent sound pattern. Motion correction: An optional fully automatic non-rigid registration algorithm is utilized to compensate for in-plane motion between the images. Clinical Indications include the ability to assess pathologic conditions that alter myocardial wa- ter content and prolong T2 relaxation times, for example, edema, acute myocarditis, heart trans- plant rejection, acute ischemia, and cardiac allograft rejection. The image acquisition typically involves the combination of black blood preparation with seg- mented gradient echo readout for a range of echo times. To generate an inline T2* map, the MyoMap software performs a pixelwise T2* estimation using a robust fitting technique, in which the signal at each TE is iteratively weighted to reflect its fidelity to a mono-exponential decay curve. Signal points farther from the ideal relaxation curve are weighted lower, reducing their influence on the fit. The weights of outlier points can be completely zero, eliminating their negative im- pact on the fit. As all echoes are acquired in a short time after a radiofrequency pulse, all multi-echo images are intrinsically registered. The curve fitting and map generation are completed inline, and require no user interaction. Please note: The T2* mapping protocols are only available for 1.5 Tesla MR systems.
