As a rule the MAMMOMAT Novation DR is put into operation from the standby mode and is then ready after a few minutes. Caution Restricted image quality due to faulty operation The detector of the MAMMOMAT Novation DR requires at least one hour's heating up time to generate an optimum image quality. Switch the MAMMOMAT Novation DR on at least one hour before using the system using the main power switch located in the room. Start-up of the system If the MAMMOMAT Novation DR has been switched off at the main power switch, you must restart the entire system. Switch the main power switch in the room on. Wait approx. 5 minutes. Switch on the PC and the monitor at the acquisition workstation. - The system changes to the standby mode. Let the MAMMOMAT Novation DR heat up for at least 60 minutes to obtain the optimum image quality. Starting the system from the standby mode Press the "Power on" button on the control console to switch on the MAMMOMAT Novation DR. Wait until "DR" is displayed on the control con- sole. Switch on the monitor and the PC at the acquisition workstation if these were switched off beforehand. Let the MAMMOMAT Novation DR heat up for at least five minutes to obtain the optimum image quality. - The internal monitoring system automatically performs a functional check of the MAMMOMAT Novation DR. - "dr" is displayed on the film density display on the control panel to indicate that the communication with the MAMMOMAT Novation DR system is functioning. Log in at the workstation. You can also immediately start with the examination, e.g. in an emergency. However, note that the image quality may be inferior. Please reference the QC section for further details and instruction. 4 Novation Basic Skills Novation Stand The Novation stand has a rotating support arm with a (FD) flat detector object table size of 24 cm X 29 cm and an object table the size of 18 cm X 24 cm (used with the Opdima®) Object table 18 cm X 24 cm used only with the Opdima (optional) (FD) flat detector ---- MAMMOMAT object table (IRSD image recep- tor holding device) including detector array Stand display Foot switch 5 Novation Basic Skills Face shield The face shield prevents the patient's head from coming into the beam path. The face shield is available in two lengths. Installing the face shield Push the face shield into the holder. Removing the face shield Bend the sides of the face shield slightly to the outside to withdraw it from the holder. 6 Novation Basic Skills Projection angle presetting knob CHE (00) -00 • Height and rotation controls are located on either side of the tube head and on both sides of the arm. The field light is located on the tube head. • The field light automatically switches on by activating the compression foot switch, or by pressing the field-light button on the tube head. • The swivel-arm system rotates between +135° clockwise and -180° counterclockwise. • The rotary motion of the swivel-arm system stops automatically at 0° and at the preset angle (+ and -), provided the movement to be performed extends over at least 14°. • The rotary motion is motorized and controlled by buttons located on either side of the tube head and on the supporting arm for the swivel-arm system. 7 Novation Basic Skills Mounting the compression plate Push the compression plate onto the pin in the compression unit. - The compression plate automatically locks in position. Removing the compression plate Push the release button and pull the compression plate out from the compres- sion unit. SIEMENS Release button MAMMOMAT Note: Mounting a proper compression paddle will engage the stand ready light. 8 Novation Basic Skills Clean the dust from the ventilation slots on the detector on a regular basis. Opfocus paddle for using on small (petite) patients. Paddle measures 20 cm X 22 cm and should be printed on 10 cm X 12 cm size film. Flat-panel detector object table (FD) The FD (Flat Detector) object table has a detector size of 24 x 29 cm and can be used for all image sizes. 9 Novation Basic Skills Maximum Force Knob F max The F max (maximum force) knob controls the force of the compression applied to the breast. The maximum force possible is 20 kg (44.1 lbs.). Siemens recommends setting it no lower than 18 kg (39.6 lbs.) Procedure 1. Set the object table to the correct height. 2. Position the breast and lower the compression plate onto the tissue. 3. Pre-compress the breast to 2-4 kg. Press on the foot switch until plate rests on the breast. 4. Perform optimized compression by pressing the foot switch again, keeping it steadily pressed until the green indicator lamp (OC) on the stand display is lit and the compression automatically stops. 5. Do not use the “pumping” technique to perform or this may override the OC feature. Important Notes on OpComp • It is important that you trust OpComp when you learn to use it. If OpComp is used correctly, you achieve an unchanging and correct compression for examination. • Some breasts might need 20 kg for correct compression. If this is more than your clinic will allow, you can always set a maximum compression force with the compression force presetting knob. • The “optimized compression” shall be regarded as a recommendation to the operator. Additional force can be applied by pressing the foot switch again. If the preset maximum compression force is less than the “optimized compression” value, the preset maximum force applies. • Patients with implants, or who recently have had breast surgery, must not be exposed to the optimized compression. Instead, use the footswitch in the normal way, or the knobs for manual compression, to adjust proper compression force on the breast. • Do not use the OpComp with flexible compression plates, spot filming compression plates, or when magnification table or the biopsy unit compression plates are used. • OpComp functions only with the compression plates of the format 18 cm x 24 cm, 24 cm x 30 cm, and the conical compression plates 20 cm x 22 cm. • OpComp is not activated by manual compression. Mounting the Magnification Table Push the guide pins of the magnification table into the side slots (Item 1) of the flat detector object table. Magnification table MAMMOMAT 12 Novation Basic Skills Setting the magnification factor You can use two different magnification factors: Push the upper part of the magnification table into the upper slot if you want to acquire the image with 1.8 fold magnification. Push the upper part of the magnification table into the lower slot if you want to acquire the image with 1.5 fold magnification. Magnification factor 1.8 Magnification factor 1.5 • Make sure the correct paddles are used with the correct height of the magnification stand. If this is incorrectly matched, the green exposure buttons on the control panel will not be illuminated. • 1.5 and 1.8 magnification factors are available. 1.8 mag 1.5 mag Spotfilming compression plate 1.5 Spotfilming compression The magnification stand has hooks designed to remove the grid from the field of view. plate 1.8 Place them in the appropriate slots and push on the magnification stand evenly. 1. Select the magnification factor by placing the table into the appropriate slot (as referred to above). 2. Use the corresponding magnification paddle. Exposure will not be possible if table slot chosen and paddle do not match. 3. To remove, pull the magnification table out from the slots and lift the rails up slightly to disengage from the detector. 13 Novation Basic Skills Mechanical Safety Possible danger areas to be aware of for protection of the operator and patient are marked in the following figures. Possible danger areas are marked in the illustration below. This will immediately stop all motorized movements of the system. STOP STOP Note: An error message will be displayed on the control panel after a short delay. Mo RR Rh Anode (Target)/Filter — Buttons for selecting the anode/filter combinations Program Auto OpDose Program Selection Program selection field for selecting four different pre-programmed radiation qualities for different breast thickness intervals. 0/1 Store button for storing exposure parameters for each breast thickness interval. Auto button for selecting and deselecting the Auto mode. In the Auto mode, the corresponding program (kV, anode/filter combination, and H/D) is suggested automatically on the basis of the thickness of the compressed breast. 18 Novation Basic Skills Manual Compression The compression plate can be moved up or down by turning the knobs on the sides of the compression unit. Knobs for manual compression/ decompression Note: In the event of power failure, decompression must be carried out manually. Automatic Decompress button On/Off button to select/deselect automatic decompression after the exposure. 0/1 mAs / mGy +/- mAs Selection and Display If the Dose Calculation option is enabled, the dose value is shown on the same display as the mAs value. After an exposure, the mAs value and the dose value will toggle in the display with a 2-second interval time, starting with the mAs value. The values remain visible until an arbitrary button on the control panel is - + pressed. The dose value is displayed in mGy (milli Gray) and represents the calculated dose absorbed in the glandular tissue in the last exposure. +/- kV Selection and Display KV If the Dose Calculation option is enabled, the kV display shows the kV value at the same time as the mAs value is shown on the mAs/mGy display. When the dose value is shown on the mAs/mGy display, the kV display shows “ “ ---- . + +/- Film Density Adjustment and Display “dr” is displayed on the film density display to indicate that the Mammomat® Novation is used. “OP” on the film density display indicates that the OpDima option is used. + **Reference Novation operating manual for further information. ** 19 Novation Basic Skills OpDose (Automatic Program Selection Mode) Program Auto 1. The unit automatically suggests the appropriate program (kVp and target/filter combination) to be used based on individual breast thickness. 2. The new program recommended by the unit is indicated by a flashing light. The operator can now decide whether to retain the current program or follow the recommendation. 3. Compression force MUST be at least 6 kg. Procedure: Auto 1. Press the Auto button to select the Auto mode. 2. Position the patient and compress the breast. 3. OpDose will now suggest a program, which will be indicated by a flashing light. This is based on the compressed breast thickness. Note: In digital mammography, on the basis of amorphous selenium, W/Rh has proven to be the preferred target/filter combination for optimized image quality. • Error messages are displayed in the ‘kVp’ and ‘mAs/mGy’ areas. They should be noted on a log. Information about these codes may be found in the service manual. • Pushing the ‘Lim’ Button on the far right will reset the control panel. After archiving, you can delete data from the local database to keep the volume of examination data stored there manageable. If you need the archived data again later on, you can re-import it using the Patient Browser. Images can be archived on CD/DVD, printed on film, or transferred to PACS. These parts are compression plates, object table, patient handles, and face shield. Cleaning • All parts of the unit that come in contact with the patient must be cleaned prior to each examination. • Wipe the unit with a wet cloth or cotton pad. For moistening, use water or lukewarm, diluted aqueous solution of household dishwashing liquid. • Do not use abrasive cleansers and organic solvents (because of incompatibility of material) or cleaning agents containing solvents (e.g., benzene, stain remover)! Caution! Do not spray the unit! The cleaning fluid must under no circumstances penetrate into the unit. Disinfection For surface disinfection, we recommend the use of isopropyl alcohol. Disinfectants based on substituted phenols and compounds splitting of chlorine are somewhat caustic and therefore, cannot be generally recommended. As a general rule, no disinfectant sprays may be used, since the spray mist may penetrate into the unit, which means that the safety of the unit can no longer be assured (damage to electronic components, formation of inflammable air/solvent fumes mixtures). Warning! As is known, some components of disinfectants are detrimental to health. Their concentration in breathable air must not exceed a legally determined limit. We recommend that the appropriate directions on use issued by the manufacturers of these agents be strictly observed. Caution! Substances, such as anesthetics and skin disinfectants, used during biopsies can damage or discolor the plastic part of the compression plates. Immediately wipe off with a wet cloth any of these substances spilled on the compression plates. Usage of M SPRAY 2000. This is to inform you that M SPRAY 2000, a spray especially made for cleaning and disinfecting of mammography compression plates, cassettes, and other inanimate hard surfaces, can be used on the Mammomat NovationDR, if it is being used in accordance stated in the “Instruction for use” and in accordance with the caution label for Siemens MAMMOMAT on the bottle. Always refer to the MAMMOMAT Novation Users Manual for complete information on cleaning and disinfection of the system and accessories. 54 Novation Basic Skills Work Status Information The work status indicates the process or editing stage that the patient and the examination data (for example, images) has reached. The work status is only displayed for data stored in the Local Database. In tree-view, it is displayed beside the object icon in the navigation area of the Browser. When image stamps are switched off, it is additionally displayed in a column of the contents area (depends on configuration). The work status consists of a 7-part string (as follows): Received/Workflow/Printed/Archived/Exported/Sent/History available Received: The 1st part shows whether data has been received over the network from other nodes, or imported from a local drive. /R/: All data received or imported /r/: Only parts received or imported / /: No data received Workflow: The 2nd part shows how far the examination and the edits have progressed.
