The following landmarks are automatically defined: Vessel ostium • Vessel ostia • Potential landing zones • The computed ostia all have a fixed initial size of 5mm (inner diameter), reflecting the common size of visceral branches. • The computed landing zones are drawn distally to the ostia (especially the lower renal ostium) and are perpendicular to the centerline. • Additionally, a C-Arm angulation associated with the landmarks is 1 cm proposed, which ensures an orthogonal (parallax-free) projection to the corresponding branch. Fusion imaging basically consists of the following steps: 1. Registration of the 3D Volume to the C-Arm geometry This ensures that the 3D volume is in the same coordinate system as the C-Arm. It can also be regarded as a ‘virtual placement’ of the 3D volume at the anatomical 2 3 corresponding position ‘in the patient’. 2. Projection of the registered 3D volume This means that the registered 3D volume is ‘virtually projected’ with the same 1 parameters as the actual X-Ray image was acquired. This is a mathematical procedure which additionally requires extensive knowledge about the current C-Arm position and imaging parameters. 3. Combined Visualization Here the ‘virtually’ projected 3D volume and the ‘actually’ projected X-Ray are displayed in the same image. During a registration process, there are three possible shift directions and three possible rotations that need to be adjusted in order to align the pre- lintnormen operative 3D dataset with the intra-operative images. Even a very accurate 3D/3D registration can be compromised by patient movement. • The accuracy of both registration types can be compromised by the introduction of stiff devices in the aorta. • Due to this potential inaccuracy it is highly required to perform an Angiography-based re-registration after the insertion of stiff devices. • Before performing sensitive operations (e.g. releasing stent grafts etc.) always confirm that the Overlay matches the anatomy. When in doubt, perform a DSA or other ways to confirm the overlay.
