MR Routine With Smart layouts a self- learning approach for best and easy definition of what data should be shown in dedicated image segments is introduced. Easy Reading functionality has been enhanced. As a new functionality when dragging and dropping images, the segment size remains the same across all segments.  When Reading with two monitors and dropping series on both monitors, the series are now synchronized for navigation and zoom on both monitors. ​ The 4D movie tool has been introduced, displaying the dynamic phases in numbers. Additionally, if scanning with GRASP and using the phase identifiers on the scanner, these identifiers are displayed.   Enlarging segments is now also possible in combination with zoom of multiple segments at the same time. This simplifies routine reading in layouts containing several segments easier.   New Image modes for enhanced visualization are available: find Cinematic VRT, Fused MIP and Fused MinIP easily accessible in the lower left corner menu or by clicking on the interactive image text of the image filter.   The Series Navigator has been enhanced. First, the series number can now be displayed directly. Second, Diffusion b-values displays the scanned b-value. Third, the echo times are displayed in square brackets for multi-echo data. Furthermore the identification of Fat, Water, In Phase and Opposed Phase for DIXON series is now easily possible by corresponding sorting into groups in the Series Navigator.   Exporting images and movies in non-DICOM formats is now available in MR applications.   A new configuration option for the export of BOLD data is available: the image data can now be sent as enhanced or non-enhanced DICOM images to the PACS system to ensure compatibility and storage. Findings Assistant The Findings Assistant is now available to automatically collect all findings made during reading. A snapshot is added for documentation as well. The details section offers you the possibility to further specify your finding (for example the type), editing its attributes, adding comments, rating the finding or trending it in follow up cases. Dedicated Breast or Prostate findings including placeholders that guide you through the measurements according to BI-RADS or PI- RADS guideline can be done. A pictogram is available for easy visualization of the findings. Furthermore Oncology findings can be done, allowing easy follow up. Prostate Biopsy Support Prostate Biopsy Support is a new functionality for preparing biopsies with ultrasound fusion systems. A fully automated segmentation of the prostate generates the volume and the contours of the prostate. Lesions can be marked, and the resulting findings and contours can be exported as DICOM RTSS and can be used for biopsy guidance.   Creating new layouts can be challenging when the scan protocols are not using the same protocol name.   To accomplish this MR Workflows are using a Self-Learning approach for best loading of the scanned series.   The system can learn based on drag and drop by the user into the layout. Alternative configurations can be used in addition to the self-learning approach.   Easy Reading is a great functionality where the images can be viewed exactly in the desired order just by image drag and drop. By dragging and dropping the layout size will always adapt to the number of dropped series. Additionally the images are automatically synchronized for zoom and navigation.   As a new functionality when dragging and dropping images the segment size remains the same across all segments. For example if you drop 5 series a layout with 6 segments is chosen, one segment would stay in this example empty.   When Reading with two monitors and dropping series on both monitors, the series are now synchronized for navigation and zoom across both monitors. Copyright © Siemens Healthcare GmbH, 2019 With Smart layouts learning image segments is done by means of examples. This learning is done by dragging a series to a specific segment and selecting Learn segments in Case Navigator.   For each example, the system stores the selected series criterion which is based on the DICOM attributes and not the protocol name. This means loading into a segment will still work even with different or changed protocol names.   The more examples you define, the higher the accuracy. The system determines the most similar series. Each series is characterized based on approx. 30 attributes that derive from the DICOM header for example MR contrast, orientation, acquisition sequence, b-value, fat saturation, Dixon, parametric map etc. Attributes of example series are compared with attributes of all series in the study. Series description is used as a fallback when the DICOM attributes cannot be read, for example with images from a different vendor. The Learning mechanism can be used only in a new site specific or private layout.   To create a new layout navigate to the Layout Gallery > Advanced Select New layout Select the desired number of segments Drag & drop the desired series to the layout segments Click on Learn Segments in top of the Case Navigator If images from a different scanner or different protocol names are available, load the images to see if the loading is happening as expected. If not drag and drop the images to the segments and click on Learn Segments again. The Smart Layout can be used in all MR Routine Workflows: MR Basic MR Prostate MR Breast MR Angiography MR Whole Spine In all Advanced Workflows it can be used in the Viewing task. For MR Cardiac Analysis it is available in MR Cardiac Reading task. When dynamic series are loaded into a segment the images can be viewed with the help of the 4D Movie tool.   In the 4D Movie tool the dynamic phases are displayed in numbers. Additionally, if scanning with GRASP and using the phase identifiers on the scanner, these identifiers are displayed in the 4D Movie tool.   For opening the 4D Movie tool go to upper left corner and select movie or press the space bar on the keyboard. In addition to normal zooming of a single segment, you can also zoom in on multiple segments at the same time. This can be very helpful especially in layouts with many segments. With the Ctrl key pressed select multiple segments. Right-click and choose Enlarge Segments from the context menu. The layout changes and all selected segments are enlarged.  Returning to the original segment size Double-click in the enlarged segments to zoom out and return to the previous layout. New Image modes for enhanced visualization are available in lower left corner. Cinematic VRT Fused MIP Fused MinIP For e.g. Angiography Cinematic VRT gives a photorealistic 3D view of the vessels.   2 series can be fused in Maximum Intensity Projection MIP, for instance fusing an angiographic series with an ADC image can give additional diagnostic value.   2 series can be fused in Minimum Intensity Projection MinIP. Easy toggle for changing the display type or the slice thickness In the segment Left click on the Image type description (MIP, MPR) toggles between the following display types MPR, MIP, VRT. Left click on the slice thickness. The slice thickness will toggle between predefined thicknesses when in MPR, MIP, VRT or Thin image type. The system automatically selects the thin mode when selecting the slice thickness. Left mouse drag on the slice thickness. The slice thickness will change by an increment of one to the dragged direction. When creating findings they are collected automatically into the findigs assistant. Per each finding a snapshot is available next to the finding.   For fast and easy viewing of all the snapshots that are listed in the findings assistant the result layout can be used. Open layout gallery and select the result layout. Exporting images and movies in non-DICOM formats is now available in MR applications. Supported image file formats: jpg, bmp, png, tiff Supported movie file formats: avi, wmf When scanning BOLD data the image data can be sent as enhanced or non-enhanced DICOM images to the PACS system to ensure compatibility and storage.   This can be changed in the configuration. All measurements made during reading are automatically collected as findings into the findings assistant. Per each finding a snapshot is available next to the finding.   It is possible to mark a single measurement as a finding or to create a multi-measurement finding out of several measurements. Findings can be assigned to specific body regions or to a specific findings type. Findings can be grouped together for example if there are different types of measurements (VOI, distance line, Mean Curve) belonging to the same lesion.   By clicking on the arrow next to the finding the details section will open where you can specify the finding. Select a measurement tool like a distance line or a ROI and draw a measurement.   The finding is listed in the Findings Assistant. Open the findings panel with the details view for editing and commenting on the findings.   For later navigation to the finding, double-click on the finding. The series and layout will adapt and display the image where the finding was created.   You can also disable automatic findings creation by deselecting the Option Automatically create findings in the Findings Assistant.   The Details section enables you to edit the finding attributes. The following shows the Details section of an MR Basic finding as an example.   In the top the type of the finding can be selected. Per default it is defined as a basic finding. This can be changed in the pulldown menu.   Editable attributes are: Location Laterality Severity Diagnosis In the lower part the overall rating can be added. Also follow up trending can be noted here. When creating measurements in the MR Breast workflow the system automatically chooses Breast as the body region and MR Breast as the findings type.   To create a breast finding, draw a measurement for example a distance line or a Mean Curve ROI. The finding is listed in the Findings Assistant.   In the Findings Assistant, once a measurement has been created the corresponding finding in the Findings Assistant automatically provides placeholders for distance measurements.   • Distance from chest • Distance from skin • Distance from nipple   Click the first placeholder Distance from chest in the Findings Assistant and then, perform the corresponding measurement in the image segment. The details view of the Breast findings enables you to edit the attributes individually for each finding, including laterality. The attributes you select for description and reporting are based on the BI-RADS standard. At the top level of all breast findings, you can edit the attributes individually for each finding and laterality. In the individual findings level you can also use the pictogram section to document the findings. The MR Prostate Workflow creates dedicated prostate findings when drawing measurements. The system automatically chooses Prostate as the body region and MR Prostate as the findings type.   To create a prostate finding, draw a measurement for example a distance line or a Mean Curve ROI. The finding is listed in the Findings Assistant.   The Gland volume to define the volume of the prostate can be calculated from: multi-measurement of three distance lines in the different planes ​prostate segmentation The Gland volume calculation from multi-measurement of three distance lines:  choose the Distance Line measurement tool from the top right corner menu and draw three distance lines by keeping the Shift key pressed:  first line for example from right to left  second line for example head to feet ​third line for example anterior to posterior Right-click the measurement and choose Calculate Gland Volume from the Context menu Welcome to the syngo.via VB40 delta online training. The details view of Prostate findings enables you to edit the attributes individually for each finding, including location. The attributes you select for description and reporting are based on the PI-RADS V2 standard. At the top level of all prostate findings, you can edit the attributes individually for each finding and laterality. In the individual findings level you can also use the pictogram section to document the findings.   The PI-RADS Version 2 requires: T2W Score (1-5) for T2-weighted imaging DWI Score (1-5) for diffusion-weighted imaging DCE Dynamic contrast-enhanced parameter (+/-) EPE Extraprostatic extension When creating measurements in the MR Oncology workflow the system automatically chooses MR Oncology as the findings type.   To create an Oncology finding, draw a measurement for example a distance line or a Mean Curve ROI. The finding is listed in the Findings Assistant.   Once a measurement has been created in the Prior study the Findings Assistant automatically provides placeholders for corresponding measurement in the current study. Click the first placeholder in the Findings Assistant and perform the measurement in the current study. The details view of Oncology findings enables you to edit the attributes individually for each finding, including trending. The attributes you select for description and reporting are based on the RECIST standard. At the top level of all oncology findings, you can edit the attributes individually for each finding and laterality. In the individual findings level you can also use the pictogram section to document the findings. In the lower part the trending values, current and baseline can be found. Prostate Biopsy Support is a new functionality for preparing biopsies with ultrasound fusion systems.   The prostate will be segmented on T2 weighted morphological images, where the morphology and lesion are well visible. The segmentation is done fully automatically and as a result the prostate volumetry and the contours of the prostate are given. Use as reference series for the segmentation only series in which the prostate has been correctly positioned in the isocenter.   The resulting findings and contours can be exported as DICOM RTSS format (RTSS = radiotherapy structure set) and used by external devices such as ultrasound fusion systems to guide the biopsy. Learn more about prostate biopsy support on the following pages. The Prostate Segmentation is calculated automatically during pre-processing. The contour of the prostate is displayed and a finding is created containing the volume of the prostate.   For viewing the segmentation click on the Biopsy Support icon in the Case Navigator. This opens a new image layout.   The segmentation of the prostate is displayed in the upper left MPR segment. Additionally a diffusion series is loaded into the upper right segment. And ADC series to the lower left segment.   The lower right segment provides tools to mark lesions.   The automatic Segmentation can be disabled in MR Prostate Workflow properties by a clinical administrator. The automatic segmentation needs to be configured in MR Prostate Workflow properties. The automatic segmentation is designed for transversal T2-weighted TSE series transversal T2-weighted SPACE series Series are identified for prostate segmentation by series description. The default configuration contains t2_tra and t2_tse_tra series description. The Segmentation will be calculated for all matching series.   Additional series can be configured for preprocessing. The images need to be T2 weighted and of transversal orientation, otherwise the segmentation might fail. If the segmentation fails due to for example sagittal orientation, a user message is shown in the message history section. The automatically created segmentation volumetry can be used as the gland volume Right mouse button click on the segmentation Calculate Gland Volume The gland volume can be calculated as in previous software versions by drawing three distance lines with the Shift key pressed. First line for example from right to left Second line for example head to feet Third line for example anterior to posterior Right mouse button click Calculate Gland Volume The automatically created segmentation can be modified. Right mouse button click on the segmentation Add region Remove region After adding or removing double click for the changes to be executed. If the automatic Segmentation is disabled for preprocessing, the segmentation can be triggered by selecting Prostate Segmentation in the upper right corner menu. In the Biopsy Support task the user can mark lesions with the tools from the lower right segment for the ultrasound guided biopsy. VOI sphere VOI freehand MR segmentation Marker The tools are also available in the upper right corner menu. The lower right segment shows the currently selected reference series and their list of findings.   When marking lesions in ADC or diffusion series, the finding will be listed in the findings assistant. To combine the findings into the segmentation of the T2 weighted image use copy and paste of the finding.   The Prostate segmentation and the lesion markers findings can be exported as a DICOM RTSS format (RTSS = radiotherapy structure set).   In the Biopsy Support task in the lower right segment: To export the findings for biopsy planning together with the corresponding reference series select Save and export. This will save the results and export them to the selected system.   Select Save to save the results locally. After completing the Workflow the DICOM RTSS is visible in the patient browser.   For series where the software cannot perform a segmentation of the prostate, for example a series in sagittal orientation or with the reference image not being T2 weighted, a user message is displayed in the message history section. Open MR Prostate Workflow properties in the MR Prostate workflow and select the tab Workflow Configuration step.   Configuration can be done for: Enable or disable automatic segmentation during preprocessing Additional series the software shall consider for automatic prostate segmentation Include or exclude the Spectroscopy task in the Workflow if the license is available  syngo.via OpenApps is a platform that easily allows download and use of additional applications from Siemens Healthineers partners on your clinical reading environment syngo.via.
