With the new AI-Rad Companion Prostate MR license, you get the Prostate Lesions functionality, which automatically detects lesions and classifies them with a score. Prostate Lesions provides several new features for the user. For example: Automatic Prostate Segmentation Automatic Lesion Detection A workflow guided layout Pre-populated results and findings and PI-RADS 2.1. reporting All of this is calculated fully automatically during pre-processing. When opening the workflow, all of these results are already available to the user for reading. Learn more about each step on the following pages. This opens a new layout that will guide you in verifying and confirming potential lesions. The layout shows the input images: T2 transversal high b-value image, or calculated b-value if available ADC map and dynamic T1 and T2 sagittal in dual monitor set up. In the lower row a fusion of the transversal T2 series and the Suspicion Map, the prostate contour, the non-peripheral zone, and contours of the potential lesions are displayed. In the Lesions preview segment, details of the currently selected lesion candidate are displayed. If necessary, adding another series from the Series Navigator is possible. The common measurement tools including Mean Curve are available. AIRC Prostate MR Input data Input data criteria For successful lesion detection, the input data must meet the following criteria: The product is still under development and not commercially available in the US. Table with 2 columns and 1 rows DWI b-values Low b-value: 0 – 100 (recommended: b=50) High b-value: ≥ 800 (recommended: around 1,000) Table with 2 columns and 1 rows Orientation of T2 and DWI series Transversal (strict transversal or anatomical) Table with 2 columns and 1 rows T2 sequences   T2 TSE or SPACE  Table with 2 columns and 2 rows Scan parameter Value Field strength 1.5 T – 3 T AIRC Prostate MR Input data [audf_sv_vb50_delta_olt_mr_airc-prostate-mr-input-data.mp3] Audio Script: For successful lesion detection, the input data must meet the following criteria: The system requires images from a 1.5 Tesla or 3 Tesla system. As a T2 sequence type T2 Turbo Spin Echo or SPACE series are used. As Diffusion Weighted images ep2d, resolve or zoomIt sequence can be used. The orientation of T2 and Diffusion Weighted series has to be strictly transversal or anatomical. From Diffusion Weighted images, at least two different b-values are required. As a Low b-value b50 is recommended, and as a High b-value around 1,000 is recommended. Based on all the input data, the system calculates a suspicion map which displays the potential lesions. AIRC Prostate MR Suspicion Map The Suspicion Map is calculated during pre-processing by a Deep Learning algorithm, using the T2 and Diffusion Weighted images as input. The potential lesions are displayed in this Map. The Suspicion Map is black if there are no lesions detected. The product is still under development and not commercially available in the US. Its future availability cannot be ensured. AIRC Prostate MR Suspicion Map [audf_sv_vb50_delta_olt_mr_airc-prostate-mr-suspicion-map.mp3] Audio Script: The Suspicion Map is calculated during preprocessing by a Deep Learning algorithm, using the T2 transversal and Diffusion Weighted images as input. The potential lesions are displayed in this Map. The Suspicion Map is black if there are no lesions detected. The Suspicion Map is a parametric map that can be fused with a morphological series. It can be sent to PACS if this is enabled in the MR Prostate Workflow Properties. If the pre-processing fails, then the Suspicion Map is not available in the Series Navigator. We will take a closer look at pre-processing requirements later in this training on the "Additional Information" page. For all potential lesions, the corresponding information on volume, region, laterality, location, score and Level of Suspicion is displayed in the preview. On the lesion tabs, the lesion with the highest score is shown first. If you select a Lesion tab, the lesion candidate is displayed in all segments of the layout. The details of the selected lesion are displayed in the Lesion preview area. They can be changed if necessary. In addition to the lesion score, a Level of Suspicion is calculated for each lesion. This Level of Suspicion provides a more granular assessment of lesion suspiciousness than the lesion score. The Level of Suspicion may support you when validating results. It will not be part of the findings information or reporting. The text ”No lesion candidates are open for validation“ is displayed when all potential lesions have been confirmed or rejected. This information is also displayed if no lesions are detected. Findings Assistant is pre-populated with findings of: Gland volume Confirmed lesions Values editable in findings details tab PI-RADS 2.1 guideline is used when filling sub scores in findings details All added findings are part of final report AIRC Prostate MR Findings and Reporting The product is still under development and not commercially available in the US. AIRC Prostate MR Findings and Reporting [vid_sv_vb50_delta_olt_mr_airc-prostate-mr-findings-reporting.mp3] Video Script: The findings assistant is pre-populated with the gland volume, confirmed lesions and lesions location. They will also be displayed in the report. The findings can be edited. In the prostate template section, the gland volume can be edited and the PSA value can be added. When adding the PSA value, the PSA density is calculated automatically. In the findings section, the findings values and scores can be edited. Each lesion subscore can be filled in. When filling in the subscores, the PI-RADS 2.1 guideline is used. The broken link indicates that this specific combination of score and subscore is not following the recommendations of the PI-RADS 2.1. The broken link is also displayed when subscores are not used. Additional findings, such as the Mean Curve, can be added using the standard measurement and reporting tools. All findings and information in the findings assistant will be part of the report, which is a one-pager. With Save and Send all findings and the report are sent to PACS, if this is configured in the Administration Portal. AIRC Prostate MR Configuration Configuration of the automatic lesion detection can be done by a clinical administrator. Perform Automated Prostate Segmentation during pre-processing. This is mandatory. Publish Suspicion Map upon Save and Send Enable Auto Detection of input series for lesion detection When disabling the automatic detection, input data definition for morphology and Diffusion Weighted series can be used The product is still under development and not commercially available in the US. AIRC Prostate MR Configuration [audf_sv_vb50_delta_olt_mr_airc-prostate-mr-configuration.mp3] Audio Script: The configuration properties can be found in the MR Prostate Workflow properties. In the MR Prostate Reading step, click the gearwheel icon to open the MR Prostate Workflow Properties dialog box. Select the Workflow Configuration tab. Properties include: Perform Automated Prostate Segmentation during pre-processing, Publish Suspicion map upon Save and Send, and Enable Auto Detection of input series for lesion detection with the series description for morphology and diffusion weighted series. For changing the configuration you need to log in as a clinical administrator. The Setting Perform Automated Prostate Segmentation during preprocessing is mandatory when using the Lesion detection functionality. The Setting Publish Suspicion map upon Save and Send is not activated by default. Upon activation, the suspicion map is sent to and archived in PACS. When the setting Enable Auto Detection of input series is selected, the input data for Morphology and Diffusion Weighted series are determined automatically. The system searches for T2 Weighted and Diffusion Weighted series, based on DICOM attributes. When deselecting this setting, a series description can be added for the series to be used. Sufficient image quality of the input image series is important. Please validate the quality prior to using images for lesion detection. Insufficient image quality can lead to incomplete or incorrect prostate lesion detection. Improper segmentation of the prostate can lead to incomplete or incorrect prostate lesion detection. Please note that the prostate lesion detection only includes potential lesions classified with score 3, 4 or 5. The Suspicion Map will not be updated if some lesions are deleted or not approved. Pre-processing may fail if the image data configured for segmentation is not available. In this case, a user message is displayed, explaining the reason for the failure. If pre-processing fails, there is no Suspicion Map in the Series Navigator, and the Preview Segment is empty.
