To start the simulation, select Practice. Navigation Help Select the icon above to open the table of contents. Click Next to continue. Next Welcome Slide The timeline displays the slide progression. Slide the orange bar backwards to rewind the timeline. Click the X in the upper right corner to exit the navigation help. Timeline Simulation Thorax CTA Examination Select Examination to perform a CT examination. Home Screen [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_home_screen_01.mp3] The Home screen is the access point for the main functions of the system. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_home_screen_02.mp3] Select Examination to perform a CT examination. Please click Examination to continue. Select Exam to load the selected patient. Select a Scheduled Patient [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_pat_01.mp3] If your system is connected to a RIS, all scheduled patients are listed in the Scheduler. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_pat_02.mp3] With the selection of the patient, the patient data are displayed on the Patient Registration dialog on the right side of the Patient Browser. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_pat_03.mp3] If your system is not connected to a RIS, use the Register Patient icon, the patient data can be entered and saved here manually. Check the patient data and medical information. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_pat_04.mp3] Select Exam to load the selected patient. Please click Exam to continue. Select the pulmonary area as desired body region. Select Thorax Body Region [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_thx.mp3] On the schematic vascular body map, select the pulmonary area as desired body region to show all available scan protocols for selection. Please click thorax to continue. Select the Thorax CTA scan protocol. Select Thorax CTA Scan Protocol [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_thx_prot_01.mp3] Dedicated Adult and Child scan protocols (CARE Child) are available. They are subdivided into Routine and Vascular scan protocols. Furthermore, special folders for clinical fields are available. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_thx_prot_02.mp3] For easier selection, a description and indication for each scan protocol is listed. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_thx_prot_03.mp3] Select the Thorax CTA scan protocol. Please select the Thorax CTA scan protocol to continue. General note: To perform a full routine examination, just follow the orange buttons highlighted in the VA40 software (guided path). Select OK to confirm and display your selection. This includes any action to take, decision to make or any button to press on the Control Box or Remote Control. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_select_ok_03.mp3] Select OK to confirm and display your selection. Please select OK to continue. Select Confirm to confirm the selected patient position and continue. Check and Confirm Patient Position [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_conf_pat_pos_01.mp3] Check the patient position and, if necessary, change it by selecting one of the eight orientation icons of the topogram taskflow card. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_conf_pat_pos_02.mp3] The patient graphic in the center of the screen indicates the selected patient position. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_conf_pat_pos_03.mp3] The selected tube position and table direction are highlighted in white. If necessary, select the appropriate icons to change them. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_conf_pat_pos_04.mp3] The correct patient position for a Thorax CTA scan should be as indicated here: The tube position set to AP or PA, the patient position set to HF supine or FF supine, and the topo length set to 512 mm, or for combined studies to 768 mm. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_conf_pat_pos_05.mp3] Select Confirm to confirm the selected patient position and continue. Please click Confirm to continue. Then select Go to activate the topogram scan. Activate Topo Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_activ_topo_scan.mp3] Then select Go to activate the topogram scan. Please click Go to continue. Select the Start key on the Control Box to start the topogram scan. Start Topo Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_start_topo_scan.mp3] Again, follow the instructions on top of the Execution Control buttons, and press the highlighted Start key on your Control Box next to you to start the topogram scan. Please click the Start key on the Control Box to continue. Select Go to activate and load the scan. Adjust Scan Range of Premonitoring Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_prem_scan_range_01.mp3] The topogram is displayed in real time and can be suspended at any time, if its length goes beyond the necessary anatomy. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_prem_scan_range_02.mp3] You are now in the Premonitoring step. On the displayed topogram, move the scan range to the correct monitoring position, in this example just above the bifurcation. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_prem_scan_range_03.mp3] Select Go to activate and load the Premonitoring scan. Please click Go to continue. Select the Move key on the Control Box to find the start position of the Premonitoring scan. Press the Move Key on the Control Box [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_press_mov_key.mp3] Check and follow the instruction below the Execution Control buttons, and press and hold the Move button on your Control Box next to you until the start position of the Premonitoring scan is reached. Please click the Move key on the Control Box to continue. Select the Start key on the Control Box to start the Premonitoring scan. Start the Premonitoring Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_start_prem_scan.mp3] The start position for the Premonitoring scan is now reached. As instructed below the Execution Control buttons, press the Start button on your Control Box to initiate the Premonitoring scan. Please click the Start key on the Control Box to continue. Select the CTA scan range to continue. Select CTA Scan Range on Timeline [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_timeline_cta_scan_range_01.mp3] You are now in the Monitoring step. The placement of the ROI (region of Interest) in the aorta is automatically done, if FAST ROI is configured. Otherwise, you will need to manually draw the ROI on the premonitoring image. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_timeline_cta_scan_range_02.mp3] Click the CTA scan range on the Timeline to be able to check and adjust its scan range. Please click the CTA scan range to continue. Select the Monitoring scan range to continue. Adjust CTA Scan Range and Return to Monitoring Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_cta_scan_range_01.mp3] To display the CTA scan range, click the dashed line around the topogram image. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_cta_scan_range_02.mp3] FAST Planning has automatically placed the correct scan range for you. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_cta_scan_range_03.mp3] If you wish to make additional adjustments to the CTA scan range, feel free to grab any of the white dots and modify the scan range accordingly. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_cta_scan_range_04.mp3] To return to the Monitoring scan, click the Monitoring scan range on the timeline. Please click the Monitoring scan range to continue. Select Go to activate and load the scan. Activate Monitoring Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_activ_monit_scan_01.mp3] Please note: You can adjust the Trigger Level in HU by dragging and dropping the purple line. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_activ_monit_scan_02.mp3] Select Go to activate and load the scan. Please click Go to continue. Select the Start key on the Control Box to start the Monitoring scan. Start Monitoring Scan [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_scan_range_01.mp3] If you haven’t done yet, connect the patient to the contrast injector and arm the injector. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_adj_scan_range_02.mp3] Then press the Start key on your Control Box, and start the contrast injector at the same time to initiate the Monitoring scan. Please click the Start key on the Control Box to continue. Select Recon All to start all configured reconstruction jobs. Check Quality of CTA Images [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_cta_images_01.mp3] You can monitor the contrast enhancement in the displayed graph and the displayed real-time image in the Tomo Segment. When the enhancement reaches the difference in HU selected, the scan will start automatically. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_cta_images_02.mp3] During scanning, real-time images will be displayed in the Tomo Segment. When the scan is completed, you can manually check the quality of the scan, be it coverage or image quality, by right-clicking, holding and moving the mouse up and down to scroll through the displayed images and check the quality of the examination. Upon receival of the images, the Check&GO algorithm is activated. The algorithm checks the anatomical coverage based on the images and the uniform and adequate distribution of contrast media, if applied. The quality check stops all auto reconstructions until the quality is approved by you. In case of insufficient quality due to anatomical coverage regions or contrast media-related factors, select Repeat Scan on the Thorax taskflow card. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_check_cta_images_03.mp3] If the quality is as expected, select Recon All in the Execution Control area to start all reconstruction jobs previously configured with Recon&GO. Please click Recon All to continue. Select Next to continue. Continue Reconstruction Step [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_contin_recon.mp3] Select Next to continue. Please click Next to continue. Select Close to close the current patient. View Dose Overview and Close Examination [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_dose_overv_close_01.mp3] If you would like to add further recon jobs, you can click the Reconstruction taskflow card. Before you close the patient examination, you’ll get a Dose Overview including all relevant dose values. [Audio file audf_ct_va40_rout_scan_wf_sim_thorax_dose_overv_close_02.mp3] Select Close to close the current patient. Please click Close to continue. Congratulations! You have successfully completed the simulation. You can try the simulation again or click Next to read the Disclaimer and exit the learning activity.
