Dimension Vista® System Sample Processing Online Training

Sufficient samples have a surplus amount of sample available for unplanned testing such as add-on tests, dilutions, and reflex testing. Samples which have insufficient volume must use appropriate sample containers, sample racks and tube adapters for analysis. To monitor sample processing status, use the Samples button in the Sidebar menu to view the real-time processing status of samples. This clinical laboratory training qualifies for continuing education units (CEU).

Interventional Ultrasound Guidance of the Ankle. Ultrasound guidance plays an important role in interventional procedures of the ankle due to nearby superficial nerves and vasculature. Commonly performed procedures include guidance of cortisone injections (most commonly into tendon sheaths or in and around scar tissue to help reduce inflammation) and PRP/autologous blood injections. This image demonstrates the peroneal tendons in an inframalleolar transverse axis. In this plane, a needle can be guided into the common peroneal tendon sheath, so bathing both tendons in one injection. This image depicts a posterior tibial tendon nerve block. The needle tip is seen abutting the posterior tibial nerve, in mid tarsal tunnel transverse orientation. The nerve is bathed with a steroid/anesthetic mixture. This can be done therapeutically, but in my experience, is more commonly done for diagnostic purposes. Here is an image of a retrocalcaneal bursal injection. Load samples into the STAT lane for priority processing. Load multiple samples into the input lane using the sample rack carrier. Unload all samples from the output lane following system identification by the Vision System and aliquoting of the sample. Navigate to the Sidebar to monitor patient sample processing. Review Patient Results Navigate to Patient Samples >All Samples >Results to review real-time patient results, dilution factors, and test report messages. Select Next to continue.   From a technical perspective, let’s consider the foot in compartments. The foot can be divided anatomically into four sections: Hind foot (talus and calcaneus) Midfoot (navicular, cuboid talus and calcaneus, and three cuneiforms) Forefoot (metatarsals and phalanges). Plantar (plantar fascia and web spaces). The Hind foot Comprises the talus and calcaneus The TTJ, the ankle joint proper, exists between the tibia of the lower leg and the talus of the ankle. Below this, the subtalar joint is located between the talus and the calcaneus. The Midfoot Comprises the navicular, cuboid, and three cuneiforms. The talonavicular joint, together with the calcaneocuboid joint, form the boundary between hind foot and midfoot The navicular-cuneiform joint is snug, and made of bone articulation on bone articulation such that only gliding movement between them The anatomy of the tarsometatarsal joint is quite complex. It consists of the articulations of multiple bones (the five metatarsals, the three cuneiforms and the cuboid bone). Injuries of the tarsometatarsal joint can be quite subtle and are not always easy to diagnose on x-ray - hence the role of ultrasound. Thirdly, the Forefoot. Comprises the metatarsals and phalanges. The MTPJs are stabilized by the plantar plate and thus if disrupted by degeneration (arthritis) or trauma, the joint is prone to subluxation or frank dislocation Synovial proliferation and joint effusion are common in arthritis. Finally, the Plantar Foot. For today’s purposes, this region consists predominantly of the plantar fascia and web spaces.  The plantar fascia is a thick band of fibrous tissue that courses along the bottom of the foot. It is attached to the calcaneus and fans out to attach to the plantar aspect of the metatarsals. The web spaces refer to the soft tissue region between the metatarsal heads in the vicinity of the inter-metatarsal nerve.   Use the Minimum Sample Guide The Minimum Sample Guide is provided to assist the user in determining adequate sample volume.  To use the guide:  Hold the sample tube against the guide for specific size tube diameter Left-side is used for aliquotted samples Right-side is used for centrifuged samples Sample volume must be above minimum sample level line for primary barcoded sample testing   Download and print a copy of the Minimal Sample Guide. To load multiple racks, use the sample rack carrier, which can hold five racks at once. When loading multiple sample racks at one time you can use the sample rack carrier.  The sample rack carrier can hold up to five racks.  Insert the carrier blade through the openings in the racks to transport multiple racks from the preparation area to the instrument. Sample racks can be loaded in the STAT input lane, where they are given priority sample processing status. Sample racks loaded in the STAT input lane are given priority in sample processing.  If you load a sample in the routine input lane that has been designated as a STAT in the LIS, it will be given priority once the vision system has identified the sample. Select Next to continue.Note: If audio does not automatically start, select the play arrow in the top left to begin.To load a sample rack, simply place it in the input lane. The input lane is indicated by the green arrow. 2The Vision System The Vision System identifies: Sample Rack Barcode Adapter Code Sample Container Barcode Sample Container Type Select Next to continue. The sample rack moves into the instrument where it stops at the Vision System. The Vision System consists of two mirrors and two cameras which take a three hundred and sixty degree picture of the rack. The vision system is used to read sample rack barcodes, adapter codes, and sample tube barcodes. It also identifies the sample container type. This information allows the Dimension Vista® System to download order requests as well as track the location of racks. 3Aliquot Probe The Aliquot Probe aspirates sample from the Sample Container. Select Next to continue.The aliquot probe aspirates sample from the sample container and positions itself over the aliquot plate. 4Aliquot Plate The Aliquot Probe Dispenses Sample into the Aliquot Plate. Select Next to continue.The aliquot probe then dispenses the sample into the required aliquot plate wells. Aliquot plates can maintain sample integrity for up to 2 hours. When using a surplus rack, autodilutions, HIL, panic reruns, reflex and add-on testing can be performed from the aliquot plate without the need to reload the original sample. The sample probe(s) can now begin testing using sample from the aliquot plate. 5Output Lane The Sample Rack exits the system via the Sample Rack Output Lane. When complete, select the X in the upper-right corner to close the window and continue.After the aliquot probe dispenses the sample into the aliquot plate, the sample rack exits the system through the sample rack output lane. The output lane can hold up to ten sample racks. An orange indicator light blinks when the output lane is full to signal the operator to remove the racks.
